Transcript Request


Mount Marty College requires a written signature to authorize the release of all transcripts. We only release official transcripts and require a $5 (per transcript) fee payable in advance.
You may request your transcript by one of the following ways:
| 1. | Our standard request form. | |
| 2. | Writing a letter including the following information: | |
| • | Full Name (former name if applicable) |
|
| • | Address | |
| • | Student ID or Social Security Number | |
| • | Name and address of where you would like the transcript(s) sent | |
| • | Your signature and date of request | |
Transcript requests may be submitted by:
| 1. | Deliver to Bede 223 |
| 2. | Mail to: MMC Registrar's Office, 1105 W 8th Street, Yankton, SD 57078 |
| 3. | Fax to: (605) 668-1618 |
| 4. | Scan and email to: registrar@mtmc.edu |
