Also include for which award you are nominating this individual: (1) Distinguished Service to Mount Marty (loyal to mission of the institution, has shown continued interest and support, and reflects honor upon the school with their Christian lifestyle), (2) Distinguished Service to Church and Community (exhibits notable leadership abilities in service to their Church and/or community at the local, state, regional, or national level), or (3) Distinguished Professional Achievement (consistently demonstrates leadership, competence, dedication, integrity, and a commitment to continued education in their chosen profession).

This nomination form and supporting information (as detailed above) must be submitted by June 15, 2013. The Alumni Relations Office will notify honorees and their nominators prior to the banquet. The awards will be presented during the All Schools Reunion weekend.
