All full time students are required to provide proof of immunization and provide a health history for the Student Health Office. These forms can be delivered, mailed or faxed. Fax number is 605/668.1524.

Failure to submit health questionnaires will result in a hold placed on future registration.

Emergency Medical Information is kept on file in the Office for Student Affairs in the event an accident should occur on campus.Please submit any emergency family contact information, medical conditions, medical allergies, insurance information.
